ONCOLOGY CAMPAIGN

1. Oncology Campaign

The Oncology Campaign is an action promoted by Joaquim Chaves Saúde, headquartered at Rua Aníbal Bettencourt, Nº3, Carnaxide, Corporate Entity and registered at the Lisbon Commercial Registry number 503904902, with a paid-in share capital of €50,000.00.

2. Object

The campaign consists of breast cancer screenings, through breast ultrasound and mammography, for the fixed price of €40, regardless of the insurance or healthcare subsystem of each person. An MD from Joaquim Chaves Saúde will be responsible for the prescriptions for the exams.

Anyone wishing to make use of other benefits related to their healthcare insurance or subsystem, should make an appointment with a doctor.

3. Campaign Duration

The campaign will begin at 10:00 on the 3^rd of October 2022, and will end at 23:59 on the 31^st of October 2022 (Mainland Portugal time).

4. Eligible Participants

The campaign is for all women 40 years old or over, residing in mainland Portugal, who are neither pregnant nor breastfeeding.

5. Campaign Communication

This campaign will be broadcast in Joaquim Chaves Saúde’s digital media and through the purchase of advertising space on digital and offline platforms; and complies with the provisions of the Advertising Code, approved by Decree-Law n º 330/90, with the last amendment introduced by Law n.º 30/2019, of April 23.

6. Data Protection

Personal data provided by the participants (full name, VAT and addresses of the Data Holders), will be processed by Joaquim Chaves Saúde as the entity responsible for their treatment, exclusively for the purpose of participating in the abovementioned campaign.

Joaquim Chaves Saúde guarantees full compliance with the European Union Regulation 2016/679 of the European Parliament and of the European Council of 27th April 2016, concerning the protection of individuals with regard to the processing of personal data and free movement of this data (“RGPD”), as well as other applicable legislation.

Customers guarantee the veracity, correctness and completeness of the data provided, and are obliged to inform Joaquim Chaves Saúde of any changes thereto.

Customers may request, at any time, the rights of (i) access, (ii) rectification, (iii) erasure, (iv) limitation, (v) opposition to the treatment or (vi) portability of their data, at any Joaquim Chaves Saúde counter.

If customers need clarification on how data will be processed, they can contact the Data Protection Officer at protecaodedados@jcs.pt.

Customers acknowledge that the exercise of the rights mentioned in the previous points may involve changes to the conditions of participation in this competition, which may make it unfeasible.

7. General Conditions

Joaquim Chaves Saúde reserves the right to change, suspend or cancel this campaign, in the event of force majeure situations, without this implying any compensation to customers.

This campaign is the sole responsibility of Joaquim Chaves Saúde. Social networks do not sponsor, and are not associated or have any intervention in the administration of the campaign, and therefore cannot be held responsible for any situation that may arise.

In the event of a situation not provided for in the regulation, Joaquim Chaves Saúde reserves the right to make any changes to the implementation of this campaign and to extend or suspend it without any prior notice.

8. Acceptance of Terms and Conditions

All customers participating in this campaign implicitly accept the terms and conditions established in this regulation and its contents.
